Dear Elementary and Middle School Parents
Illinois provides a program called Miles of Smiles which may provide an exam, cleaning, fluoride treatment, and sealants (a protective coating on chewing surfaces on back of teeth). This program is open to all students regardless of ability to pay. If your child does have dental insurance or is a member of the All Kids Program, the service is billed to those companies so please fill out the requested information on the Miles of Smiles form. Parents will not receive a bill.
The clinic will be set up in a conference room at GCMS Elementary. Transportation to the clinic will be provided. To ensure enough supplies and staff are assigned to our clinic, we must send the company the number of students participating. If you are interested in signing your student up for this clinic, please only fill out the correct side and return this enrollment form to the office by Thursday February 2nd, 2023.
