The High School Golf team will have tryouts on Monday, August 9 at 9:00am at Railside Golf Course in Gibson City. Athletes must turn in the following forms in order to participate:
-GCMS Athletic Insurance Verification Form
-GCMS Concussion Information Form
-IESA/IHSA Physical Exam Form (must be signed by a doctor)
-2021-22 District COVID Permission form for athletics
-proof of insurance (copy of insurance card or letter from insurance company)
Completed forms can be brought to tryouts or emailed ahead of time to Coach Ann Spangler at aks@noradltd.com
